Program Description


The Community Health and Case Management Certificate prepares students to work on the frontline in the fields of public health, health care and human services. Coursework is focused on the theoretical knowledge, ethical practices and core competencies needed to assist individuals and families in need of medical, mental health and social services. Students will learn how to work with their clients in creating service plans, identifying resources, and developing strategies for the implementation of services. The certificate includes courses and fieldwork where students learn to apply their knowledge and skills in real work environments. Employment opportunities are available in numerous health and human service settings including hospitals, community health centers, mental health centers and substance abuse centers.

Those who complete their certificate and decide to continue their studies may transfer their completed course credits into the Health Studies A.S. , Human Services A.S. , or Human Services A.A.S.  

Program Learning Outcomes


Upon successful completion of this program, a student will:

  1. Demonstrate an understanding of the theoretical knowledge base required for employment as a community health worker or case manager.
  2. Apply ethical practices in the workplace.
  3. Solve problems by selecting and applying information resources, logical thought processes and problem- solving methodologies.
  4. Demonstrate cultural self-awareness and apply appropriate strategies in working with culturally diverse populations.
  5. Practice advocacy skills in the following domains: health care access, health care system navigation and care coordination.
