Program Description
The Community Health and Case Management Certificate at SUNY Westchester Community College prepares you for essential frontline roles in public health, healthcare, and human services. Through courses in healthcare systems, case management, and social services, along with hands-on fieldwork, you’ll develop the knowledge and practical skills to support individuals and families in accessing and coordinating medical, mental health, and social services. This program emphasizes ethical practices, cultural sensitivity, and advocacy, equipping you to work effectively with diverse communities in settings like hospitals, community health centers, and mental health facilities. Additionally, credits from this certificate can be applied toward further degrees in Health Studies or Human Services . Program Learning Outcomes
Upon successful completion of this program, a student will: - Demonstrate an understanding of the theoretical knowledge base required for employment as a community health worker or case manager.
- Apply ethical practices in the workplace.
- Solve problems by selecting and applying information resources, logical thought processes and problem- solving methodologies.
- Demonstrate cultural self-awareness and apply appropriate strategies in working with culturally diverse populations.
- Practice advocacy skills in the following domains: health care access, health care system navigation and care coordination.
