HCTAL 104 - Community Health Worker Foundations

The Community Health and Case Management Certificate prepares students to work on the frontline in the fields of public health, health care and human services. Coursework is focused on the theoretical knowledge, ethical practices and core competencies needed to assist individuals and families in need of medical, mental health and social services. Students will learn how to work with their clients in creating service plans, identifying resources, and developing strategies for the implementation of services. The certificate includes courses and fieldwork where students learn to apply their knowledge and skills in real work environments. Employment opportunities are available in numerous health and human service settings including hospitals, community health centers, mental health centers and substance abuse centers.
